Definition of ACT The simple definition of assertive community treatment is an intensive, integrated approach to community mental health service delivery. What this means is that mental health services are provided in a community setting (rather than a more restrictive residential or hospital setting) to people experiencing serious mental illness. The mission of ACT is to help people become independent and integrate into the community as they experience recovery. Secondary goals include reducing homelessness and unnecessary hospital stays. In this way, ACT offers treatment in the real world and the team of professionals provides help using a whole team approach. Many people suffer from symptoms of mental illness that impact their ability to function in daily life and that land them in the hospital emergency department seeking services. The goal of ACT is to reduce this reliance on hospitals by providing round-the-clock services to the people who need it most. In this way, assertive community treatment could be expected to help to reduce preventable outcomes of mental illness, such as homelessness and substance abuse. History How did assertive community treatment get its start? Go back to the 1970s and a picture will quickly emerge of a shift away from institutionalization of patients with severe mental illness. At the same time, community services were poorly set up to help these people who were no longer living in institutional settings. The founders of ACT were Leonard I. Stein, Mary Ann Test, Arnold J. Marx, Deborah J. Allness, William H. Knoedler and other colleagues at Mendota Mental Health Insitute in Madison, Wisconsin (a state psychiatric hospital). As unusual as it sounds, at the time that assertive community treatment first came into being, it was considered a radical approach. Since that time, however, it has become one of the most important delivery methods for specific populations of those with mental illness. Whats more, the National Alliance on Mental Illness (NAMI) has been on board, publishing a manual for ACT in 1998 and operating a training and advocacy center from 1998 to 2004. Who Assertive Community Treatment Serves If you or a family member has been assigned ACT services, you might wonder why you were chosen to receive this type of service. Services are not provided in a clinic, but rather in the patients home, in community locations (such as a coffee shop, restaurant, or public park), in homeless shelters, or wherever is most appropriate and convenient for the person using the service. Characteristics If you are about to enter assertive community treatment, you are probably unsure of what to expect. Most ACT programs have similar structures, so the following may give you some guidance on what the program will offer. Your treatment plan will be centered around your own personal strengths, needs, and desires for the futureACT is offered long-term but not unlimited; the goal is to eventually transition you to other services or for you to maintain what you have learned on your ownThe focus of ACT is on community integration and getting you back to living a normal lifeACT follows a holistic approach to treatment, meaning that all areas of your life are targets for improvementACT usually has a ratio of about 10:1, meaning that for every 10 clients there will be one team member assignedMost clients have multiple contacts with team members each weekACT is offered 24 hours a day, 7 days a week to ensure that you always have the help you needAn ACT team generally includes a psychiatrist, social workers, nurses, occupational therapists, peer support specialists, and moreACT is a considered a medically monitored non-residential service, which is two steps down from the highest level of service (medically ma naged residential service)Team members will develop long-term relationships with you regardless of whether you become homeless or find yourself incarceratedWhen you are doing better, you will be transitioned out of the ACT program Services Provided by ACT What are the specific services that you can expect to receive from the ACT team? The following is a list of some of the primary services that assertive community treatment offers: initial and ongoing assessmentspsychiatric services such as coping with psychotic episodes or crisessubstance abuse serviceshelp with employment and housingeducation for family members and you about your mental illnesstreatment planning and monitoringaccompanying you to medical appointments and dentist appointmentsrepresentation at court hearingshelp with finances (such as managing money and paying bills)helping you to grow confident with housekeeping, shopping, cooking, and transportationemployment support such as finding and keeping a jobhelp to order and manage medications The Benefits Overall, research evidence on assertive community treatment has been positive with some caveats. A 2016 evidence review showed that ACT reduced self-reported psychiatric symptoms, hospital stays, and emergency department visits among people with mental illness and substance abuse. In general, from the dozens of randomized controlled trials that have been conducted, it can be concluded that ACT is more effective than standard services in reducing hospital use, but findings for other outcomes are less clear. While studies have shown improvements in housing, symptom management, and quality of life for ACT, these findings were most evident for people who tended to return to hospitals often. It has been noted that ACT may be most helpful in communities that do not have well coordinated mental health care systems, which result in overuse of hospitals by those with serious mental illness. For example, ACT may show better outcomes in the United States than in the United Kingdom, because the former has a poorly coordinated system for mental health care that results in more hospital stays, while the latter has a well-coordinated system that already includes many aspects of ACT in its standard care. Other research has shown ACT to be more effective than standard care in reducing the risk of hospital stays and incarceration, specifically in poor inner-city neighborhoods, and reducing alcohol use or incarceration among those with antisocial personality disorder. Furthermore, among the homeless, it has been shown that ACT results in more satisfaction with care and more stable housing situations than standard care. This is easy to understand when you consider the ACT modelâ€"homeless individuals meet service providers where it is convenient for them are more likely to benefit from services than those who must travel to receive help. Finally, in a Cochrane review of 38 clinical trials from the United States, Canada, Europe, and Australia, it was shown that ACT was more effective than standard care in lowering the time spent in hospitals and improving social functioning and independent living. At the same time, mental health and quality of life were not shown to improve more than when standard care was offered. Overall, the research evidence suggests that ACT is most helpful for people with serious mental illness as a method to keep them out of the hospital and in the community. Criticism of ACT Overall, there have been some criticisms of the ACT program. One is that ACT is simply a system of coercion in which hospitals deny admission to patients based on their enrollment in the ACT program. At the same time, isnt staying in the community the goal of all mental health initiatives? In this way, it seems that regardless of whether the tactics are coercive, if patients can be treated in the community where they live, tracked through prison and homeless shelters, and offered 24/7 care at their convenience, then the benefits may be worth the drawbacks. While it may not necessarily be the fault of the teenagers themselves, rather their parents instead, the opinion is still there. Along with this opinion is the assumption that you cannot trust teenagers with big, cumbersome responsibilities, they can’t handle it. While teenagers can be emotional and at times irresponsible, that doesn’t mean they cannot make a difference or have an effect on history. In some places in the world teenagers and even preteens are forced into marriages and responsibilities that teenagers of the western world just wouldn’t be able to handle. In order to make the separation between the two groups official, Thomas Jefferson wrote the Declaration of Independence. Two hundred years later, women were facing the same injustice, only it was from men; and to protest against the unfair treatment, Elizabeth Cady Stanton wrote the Declaration of Sentiments and Resolutions. Both parties of oppression fought against the same outcome, unjust treatment, yet, each fought against different causes and in different ways. Jefferson and Stanton direct their piece to different audiences. Jefferson claims in his piece that,†¦show more content†¦He does not just declare independence, he also declares war. Stanton uses the template of the Declaration of Independence to carry out the same tone of seriousness. She informs the audience that the issue between men and women is as serious as the issue between the King and the Colonies. What is ethnicity? Ethnicities are the common characteristics of a group of people. As a large multi-national country, China is composed of 56 ethnic groups. My family is Han Chinese. Therefore, my roots allow me to be classified as Chinese. Mandarin is my mother tongue, all my families speak it and read it. All education is conducted in Mandarin. It does not mean there no problem among the Mandarin speakers all around the China because there are over 600 regional dialects except Mandarin. Older generation used to speak regional dialects, like my parents often speak Fukienese with my grandparents. Generally, China’s main religion are Buddhism, this was the only foreign religion to be absorbed into and changed by Chinese culture, contributing in many ways to the country’s cultural development. Buddhism is also my family’s beliefs, we will worship Budda in some festivals. No matter the language or the religion also help to identify me as full-blooded Chinese. Other identifying me as a Chinese by my skin color, hair, eyes, other appearance features, and ask me where I come from. I’m a little bit shy at school, and when I have questions or I need help on my subjects, I hesitate to raise my hand. Since my freshman year I have been like this. I attend every class and turn in every homework or project on time, for this reason, they identified me as an Asian, because of the stereotypes placed on Asians my behavior and attitude contributed to people’s reasoning. China is the oldest civilization in the world and Chinese civilization was built on agriculture. The collective (group-oriented) nature of Chinese values is largely the product of thousands of years of living and working together on the land (Hu, Grove 1) Today, China is a communist government with a socialist ideology and a capitalist economy. Traditional festivals are celebrated in many countries all around the world. China with its long history and predominantly agricultural society has large number of traditional festivals. As Eberhard states â€Å"Few nations have such a multitude of romantic and colorful celebrations as do the Chinese† (Eberhard 31). Also, the family not the individual, is the basic unit in China, all festivals are family festivals rather than church or state festivals. The Chinese New Year was and still the most widely celebrated festival throughout the whole China. It is the time for family reunions, a time for visits with friends, a rest for the hard work and all sorts of entertainment. That is so true that The Chinese New year is a big event and it should be my favorite festival. The Chinese New year officially lasts one month, but in that very day everyone tries to be at home, families sit all together at a big table to share a delicious dinner. Kids wear new cloths and wait for pocket money from parents, then we have firecrackers to welcome the New year. Usually, the first day of the New Year is devoted to feasting and visiting relatives. On the second or third day friend visit and exchange good wishes. Food is one of the most significant resources in life. All cultures differentiate themselves through their development and attitude toward food. Therefore, there is a wide variation of food, which often has a lengthy developmental history, that symbolizes diverse cultural attitudes; specific foods, such as pasta for Italians, curry for Southeastern Asians, and pierogi for Poles, serve as a focus for ethnic identity (Hirschberg 57). As a Chinese, I respect all food, and do care about it. Maybe I could say it is the most distinguished cuisine in this world. Americans like pastas, pizza or meat as entrees, however rice is a basic and important food to us. People in western countries like to use butter, cheese as ingredients, but we use soy sauce and bean curd. We have so many different techniques for cooking food, such as boiling, deep-frying and pickling, but they are not always done in the totally same ways. Take an example of boiling, one is cooking in water makes it continues to boil, and the other one is put the stuff in boiled water and then reducing the temperature. However, although Chinese people share the same culture, some food items a re available in only particular regions. I lived in south China, there are many basins and plains, which help the various harvests of food because of the warm and moist growth environment; rice, green vegetables, and animals are produced. Do not like northern China, most likely are grown for wheat. As the globalization, Chinese and Americans are interacting with each other more. The western traveler must be aware of the different greeting styles that exist in China. The most common greeting among the Chinese is Nihao, which means greetings or good day. We do use direct greetings like â€Å"Good morning†, â€Å"Good afternoon†, or â€Å"Good night†. You may use Nihao either on virtually any occasion regardless of the time of day or the social status of the person you are greeting. And the most conventional way to greet a Chinese is simply to say his or her name, perhaps adding a term of respect. For examples, a girl just back from school may call out â€Å"Baba† (Daddy) when greeting her father. Prepare for this paper I searched for several materials and look up so many book about the Chinese culture. As a Chinese I still learn lots of new acknowledge that I never know before. For instant, like Chinese New year, I did not know the origin of this festival. I would like to share information with others but I don’t want to make a label with racism or others. Tobias Smollett Biography Tobias George Smollett (1721-1771), Scottish novelist, was born in Dalquhurn, Dumbarton County Scotland. Smollett was born beneath a plane tree at Dalquharn House on the family estate of Bon hill in the Vale of Leven, near the village of Renton, Dumbartonshire. At fourteen Smollett was apprenticed to a Glasgow doctor. He studied medicine at Glasgow University and moved to London in 1740. He was a ship's surgeon in the Carragena expedition against the Spanish in the West Indies, and lived in Jamaica until 1744 when he returned to London and renewed his earlier attempts to stage a play he had written The Regicide, but still met with no success. He also failed to set up his own medical practice. His first novel, the partly autobiographical Roderick Random (1748), was an immediate success. His best novel, The Expedition of Humphry Clinker (1771), has become a classic. It is a story, told in a series of letters, about the travels of a family through England and Scotland. Smollett was troubled by lack of money. He spent his last years in poor health, and died in Livorno, Italy, on October 21, 1771. Two years later, Johnson and Boswell stayed at Cameron House with Smollett's cousin James, who was preparing to erect a Tuscan column in Smollett's memory at Renton. Johnson helped compose the Latin obituary on the plinth, and the column stood in what subsequently became the playground of a school. Some critics regard Tobias Smollet as more satirist meaning that a work of literature or art that, by inspiring laughter, contempt, or horror, seeks to correct the follies and abuses it uncovers. I don't know what that means though. This is a paragraph from Tobias Smollett's book The Adventures of Roderick Random. Roderick Random is the orphaned, unwanted grandson of a severe old Scots magistrate, exposed by his grandfather's known neglect to the malice of the community. His principal enemies are the schoolmaster and the young heir. It is not long before a deus ex machina appears in the form of a sailor uncle: He was a strongly built man, somewhat bandy-legged, with a neck like that of a bull, and a face which had withstood the most obstinate assaults of the weather. His dress consisted of a soldier's coat, altered for him by the ship's tailor, a striped flannel jacket, a pair of red breeches japanned with pitch, clean grey worsted stockings, large silver buckles that covered theree-fourths of his shoues, a silver laced hat whosecrown overlooked the brim about an inch and a half, a black bob wig in buckle, a check shirt, a silk hankerchief, a henger with a brass handle girded on his thigh by a tarnished laced belt, and a good oak plant under his arm.
